We don’t just play the same old Bingo games! Nestled on a scenic hilltop in Troy, Eddy Heritage House boasts beautiful sweeping views of the valley. Located just off Hoosick Street and Rt. 787, our easy accessibility makes it convenient to visit a loved one from anywhere in the Capital Region.
Eddy Heritage House offers a robust short-term sub-acute rehabilitation program as well as a long-term skilled nursing care program. The 40-bed rehab program provides intense therapy to help patients with aggressive rehab goals achieve positive outcomes from strokes, hip and knee replacements, and other conditions. We can return you to your home, your community, and your life.
For our long-term care residents, our activities department offers a wide variety of engaging events to stimulate residents, from leaf-peeping trips to Turkey Trots, shopping outings, and simply great entertainment!
Our campus also offers plentiful outdoor space with a large front porch and back garden areas where residents love to relax and enjoy Mother Nature, or visit with family and friends.

A message from the executive director
Eddy Heritage House strives to provide compassionate care to our residents and our home-like environment makes it feel like we’re caring for and working with family. We always welcome the opportunity to work with your family and we welcome your participation in your loved one’s plan of care so together we can achieve successful outcomes! All of our team members here are supported by the St. Peter’s Health Partners organization and are encouraged to perform at their highest level every day!
- Sherri Mier
[ Sherri has over 35 years of healthcare experience. A registered nurse for nearly all that time, her specialties include infection control and geriatrics. Eddy Heritage House residents and patients benefit from the combination of healthcare administration and nursing background she brings to her position. ]
Contact MeMedicare has awarded us one star overall, one star for health inspections, four for staffing and one for quality measures. We encourage you to contact us with any questions about what they mean or how they were determined.
As of March 1, 2025, our rates are as follows:
- $546 per day for a private room in long-term care.
- $521 per day for a semi-private room in long-term care.
- $680 per day for a private room in short-term rehab.
- $641 per day for a semi-private room in short-term rehab.
**Room rates DO NOT include applicable New York State Assessment Tax.
Ownership: Eddy Heritage House is proud to be part of St. Peter’s Health Partners, a non-profit health system which is licensed as a co-operator of our skilled nursing residence. SPHP’s parent, Trinity Health, is licensed as a co-co-operator. Both also provide us with administrative services.
Landlord: Eddy Heritage House/St. Peter’s Health Partners owns our building and property. We do not rent or lease; we do not have a landlord.
Vendors: This is a current list of all the vendors with whom we contract to provide us with goods or services. We update this within 30 days of executing any new agreement or contract.
